Comparison of SARS-CoV-2 serological tests with different antigen targets

These last months, dozens of SARS-CoV-2 serological tests have become available with varying performances. A major effort was completed to compare 17 in April 2020 Switzerland. In a preliminary phase, we compared IgG, IgM, IgA and pan Ig including ELISA, LFA, CLIA ECLIA on panel 182 sera, comprising 113 sera from hospitalized patients positive RT-PCR, 69 sampled before 1st November 2019, expected give negative results, respectively. second the five best performing most were further evaluated total 582 (178 404 negative, respectively), allowing assessment 20 possible cross-reactions other viruses. among eight IgG/pan-Ig ELISA or CLIA/ECLIA tests, had sensitivity specificity above 90 % 98 respectively, six IgM/IgA only one acceptable. Only LFA test three showed good performances for both IgG IgM. For all IgM aroused concomitantly. no particular cross-reaction. We observed an important heterogeneity development antibody response. The majority exhibited high detect response moderately critically ill patients. mostly insufficient added value early diagnostic cohort tested this study.
